Grant Application Procedure:

Deadlines for submission of grant proposals for 2024 are February 15, June 14 and October 15.

The Grants Committee reviews proposals and recommendations are made to the Board of Directors for approval. Staff and/or Grants Committee members may visit the applicant organization as part of the review process.

The Foundation will prepare all copies for the Committee. For this reason, proposals should be turned into the Foundation office unbound, using only paperclips or binder clips (please do not staple any portion of the document).

Grant proposals for programs or projects should include the following, if applicable:

1. Title page 

Organization’s name, address, and telephone number

Contact person and title

Project/program title and amount requested

Date of application

 

2. Proposal narrative 

Issues project/program will address

Expectations of project/program

How many will benefit from project/program

Role for volunteers in project/program

Plan for evaluation of project/program

Signed endorsement of the proposal by the organization’s Board of Directors

 

3. Financial information

Budget for proposed project / program, including income and expense data

Minimum of two pricing quotes for equipment requested

Listing of other funding sources applied to and / or received for project / program

If applicable, how project / program will be funded in future

 

4. Organizational information

Brief history of organization, including mission and purpose*

List of officers and members of governing board, including addresses and telephone numbers

Year-end financial statement or audit; if more than six months since statement or audit, also include most recent financial statement

Copy of tax exemption from Internal Revenue Service*

 

*If the organization has previously applied to the Foundation for a grant, it is not necessary to resubmit this information unless a fundamental change has occurred within the document(s).
